Job summary

Angels Care Home Health seeks a licensed Physical Therapist to plan, implement and evaluate patient care plans for home health care. You will develop PT treatment programs from referrals, monitor progress, and adjust plans to restore or maintain patient wellbeing. Collaboration with an interdisciplinary team is essential to provide high-quality care.

Qualifications

• Current state license as a Physical Therapist.
• Two years of experience as a Physical Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Plan, implement and evaluate patient care plans to restore or maintain patient wellbeing.
• Plan, organize and conduct physical therapy treatment program based on medical referral and evaluation.
• Collaborate with interdisciplinary team.
