Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist (PT) for part-time / per diem home visits in Massachusetts. The PT will evaluate clients at home, develop personalized care plans, and provide interventions to improve strength, balance, mobility, and safety while educating families and collaborating with the care team.

Required: Active MA PT license, DPT or MPT, CPR certified, and 1+ year experience preferred; new grads welcome.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certified.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using a mobile-friendly EMR.
