Job summary

Legacy Healthcare Services is seeking a Physical Therapist to provide compassionate rehabilitative care to senior residents in our not-for-profit communities. You will plan and implement individualized treatment plans based on clinical findings, working with a coordinated care team to improve quality of life.

The role offers a Monday–Friday schedule with no weekend or holiday requirements, and opportunities for professional growth.

Qualifications

• Proven ability to build relationships with patients and families.
• Strong clinical assessment and treatment planning skills.
• Comfort with electronic records and basic software tools.

Responsibilities

• Provide rehabilitative services to senior residents as part of a coordinated care team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the plan of care.
• Deliver one-on-one sessions with residents using functional-based approaches.
• Adapt sessions to individual needs while maintaining treatment quality.
