Job summary

Legacy Healthcare Services is seeking a Physical Therapist to join our not-for-profit senior living rehabilitation team. You will provide individualized rehabilitative services within a private treatment setting, plan care with clinicians, and participate in conferences to coordinate plans.

• Provide rehabilitative services to patients based on clinical orders.
• Develop initial and ongoing treatment plans from clinical findings.
• Participate in clinical conferences to coordinate care plans.
• Deliver one‑on‑one treatment with a functional-based approach.

• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to share and coordinate the plan of care for patients.
• Treat residents one on one, not in a group type setting, so each patient gets individualized attention through private treatment sessions with a functional based treatment approach.
• You have the ability to individualize treatment sessions!

• Degree in area of discipline
• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment
