Job summary

BrightStar Care of Lubbock is seeking a Physical Therapist to provide in-home PT services in the Lubbock, TX area. The role involves evaluating patients, developing personalized treatment plans, and delivering care in patients' homes, with a focus on mobility, strength, and independence.

The position supports both PRN and full-time schedules with a focus on comprehensive patient care and collaboration with the care team.

Qualifications

• Active, unrestricted Texas Physical Therapist license.
• Minimum 1 year of PT experience.
• Current BLS/CPR certification.
• Own device and reliable internet connection.
• Valid driver's license and reliable transportation.
• Strong communication, organization, and clinical skills.
• Ability to work independently and manage patient care.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in patients' homes.
• Address strength, balance, mobility, and functional goals.
• Monitor progress and modify treatment plans as needed.
• Collaborate with PTAs and other members of the care team.
• Work with patients across a variety of ages, diagnoses, and needs.
• Complete accurate and timely clinical documentation.
