Silver Creek Physical Therapy, part of Therapy Partners Group, seeks an outpatient Physical Therapist to join a premier orthopedic care team serving San Jose and surrounding communities. You will evaluate patients, design personalized treatment plans, and apply evidence-based modalities in an outpatient setting.

Qualifications

• DPT from an accredited program.
• Current state licensure or eligibility for licensure as a Physical Therapist.

Responsibilities

• Conduct comprehensive assessments of patients’ physical conditions, mobility, and functional abilities.
• Develop individualized treatment plans based on evaluations and patient goals.
• Implement various therapeutic techniques, including manual therapy, exercise prescriptions, modalities, and patient education.
• Maintain accurate and timely documentation of patient progress using Raintree.
• Educate patients and families about rehabilitation processes, home exercise programs, and preventative care.
• Regularly assess and adjust treatment plans based on patient progress and feedback.
• Stay updated on latest research, techniques, and best practices through continuing education.

We are seeking a dedicated and skilled Outpatient Physical Therapist to join our dynamic team. The ideal candidate will provide high-quality patient care, develop tailored treatment plans, and help patients achieve their rehabilitation goals in an outpatient setting.

Key Responsibilities:

• Patient Evaluation:Conduct comprehensive assessments of patients’ physical conditions, mobility, and functional abilities.
• Treatment Planning:Develop individualized treatment plans based on evaluations and patient goals, considering evidence-based practices.
• Therapeutic Interventions:Implement various therapeutic techniques, including manual therapy, exercise prescriptions, modalities, and patient education.
