Silver Creek Physical Therapy, a member of Therapy Partners Group, seeks a dedicated Outpatient Physical Therapy Assistant to join a collaborative orthopedics team serving San Jose area for over 20 years.

Responsibilities include assisting PTs with treatment plans, performing exercises and modalities, documenting progress in Raintree, and educating patients. Licensure eligibility and strong patient care are required, with benefits including health insurance and 401(k) matching.

Qualifications

• Associate degree as Physical Therapy Assistant from an accredited program.
• Current state licensure or eligibility for licensure as a Physical Therapist Assistant.
• Outpatient experience preferred but not required.
• Ability to work collaboratively in a team-oriented environment.
• Commitment to providing exceptional patient care.

Responsibilities

• Patient FollowUp: conduct therapeutic exercises and modalities as instructed by the PT.
• Treatment Planning: assist PT in developing and implementing patient treatment plans.
• Therapeutic Interventions: implement manual therapy, exercises, modalities, and patient education.
• Documentation: monitor and document patient progress during treatment sessions using Raintree.
• Patient Education: educate patients and families on exercises and pain management.
• Professional Atmosphere: work with healthcare team to ensure patient safety and quality care.

Pay: $40-44/hour depending on experience

We are seeking a dedicated and skilled Outpatient Physical Therapist Assistant to join our dynamic team.

Key Responsibilities

• Patient FollowUp: Conduct therapeutic exercises and modalities as instructed by the physical therapist.
• Treatment Planning: Assist physical therapists in developing and implementing patient treatment plans.
• Therapeutic Interventions: Implement various therapeutic techniques, including manual therapy, exercise prescriptions, modalities, and patient education.
• Documentation: While using Raintree, Monitor and document patient progress during treatment sessions, reporting changes to the physical therapist.
• Patient Education: Educate patients and families on exercises, pain management techniques, and care plans
• Professional Atmosphere: Work collaboratively with the healthcare team to ensure optimal patient care and safety.

Qualifications

• Associate Degree as Physical Therapy Assistant from accredited institution.
