Job Overview
The Physical Therapist – PACE position provides high quality services that help restore function, improve mobility, relieve pain and prevent or limit permanent physical d isabilities of patients suffering from injuries or disease as they restore, maintain and promote overall fitness and health. Responsible for evaluating individuals with mobility dysfunction and providing a plan of care that is specific to the individuals goals related to functional independence.
Physical Therapist (Corporate)
This is a specialized Physical Therapist (PT) position to support the Rehabilitation Manager directly ove r five major domains of work: staff coverage, staff training, administration, project coordination, and regulatory compli ance. The PT will cover various PACE Physical Therapist duties remotely or in-person for any of the AltaMed PACE Centers, as needed to support compliance-related needs such as assessments, care planning, and IDT/Care Coordination me etings. The PT will support training such as onboarding, process implementation, and compe tency re-training as assigned by the Rehabilitation Manager. The PT will support Rehab operations by assisting process es such as data collection, data analysis, report building, preparing and facilitating presentations, and compliance recordkeeping. The PT will support PACE by assisting with program planning and implement ation, participating in CQI initiative s, and serving on interdisciplinary committees to improve participant health outcomes and the delivery of Rehab services. The PT will co llaborate with discipline managers, QI team , the Rehabilitation department, and other PACE lead ers to align with organizational goals, provide exceptional customer experience and optimize care delivery.
Physical Therapist license issued by the State of California Physical Therapy Board coupled with a minimum of 1 year of experience working with the frail elderly (Geriatric) and disabled populations preferred.
Current BLS certification is also required .
