• Use standardized assessments and evidence-based practice to guide interventions.
• Provide rehabilitative, skilled, and medically necessary treatment per regulations.
• Administer procedures such as ADL retraining, therapeutic activities, exercises, modalities, and assistive devices.
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and modify services based on patient response.
• Provide counseling to patients, families, and caregivers.
• Communicate progress and barriers with supervisors and the health team.
• Prepare comprehensive discharge summaries per clinical requirements.

PHYSICAL THERAPIST – PRN – WEEKEND AVAILABILITY

Alamitos West Health & Rehabilitation in Los Alamitos, CA, is seeking a PRN Physical Therapist with weekend availability to join our in‑house therapy team.

Responsibilities

• Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plan of care following all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan including but not limited to ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
• Assess patients’ progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness; modify services based on patient evaluation and referral appropriate outcomes.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
• Effectively communicate with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.

Qualifications

Licensed Physical Therapist.
