Upland Rehabilitation is a post‑acute care facility located in Upland, CA offering long‑term, short‑term, and geriatric services. We are seeking a PRN Physical Therapist to provide weekend coverage for our in‑house rehabilitation program.

Pay: $55–$62 per hour, based on experience.

Duties

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans in accordance with regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, following regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices, as well as wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
