Oasis Therapy Services in Wisconsin seeks a full-time Director of Rehabilitation (DOR) to provide one-on-one physical therapy and coordinate therapy operations across Greendale and Waukesha.

As DOR, you will supervise therapists and assistants, ensure regulatory compliance, collaborate with physicians, nursing, and caregivers, and supervise documentation in Net Health to support quality resident care.

• Active Wisconsin Physical Therapist license and NPTE clearance.
• CAPTE-accredited PT education and CPR certification.

• Provide one-on-one PT interventions to promote safety and independence.
• Coordinate daily rehab operations across Greendale and Waukesha locations as Director of Rehabilitation.
• Supervise Physical Therapist Assistants and staff in accordance with Wisconsin regulations.
• Maintain documentation and regulatory compliance; support quality improvement initiatives.
• Collaborate with physicians, nursing, and care teams to coordinate care and ensure outcomes.

Oasis Therapy Services is seeking a full-time Physical Therapist (PT) to serve as Director of Rehabilitation (DOR), providing one-on-one rehabilitation services and coordinating therapy operations across facilities in Greendale and Waukesha, Wisconsin.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans addressing mobility, strength, balance, pain management, functional limitations, and fall prevention.
• Provide skilled one-on-one physical therapy interventions to promote residents' safety, mobility, and independence.
• Serve as Director of Rehabilitation, coordinating daily rehabilitation operations, staff schedules, therapy services, and interdisciplinary communication across both locations.
• Provide clinical supervision of Physical Therapist Assistants and other appropriate personnel in accordance with Wisconsin physical therapy regulations.
• Monitor residents' progress, perform reassessments, and modify treatment plans as clinically indicated.
• Support rehabilitation staff with documentation standards, regulatory compliance, and quality improvement initiatives.
• Collaborate with physicians, nursing staff, residents, caregivers, and other healthcare professionals to coordinate care.
• Maintain accurate and timely documentation, including evaluations, progress notes, treatment records, and discharge summaries in Net Health.

Qualifications

• Active and current Wisconsin Physical Therapist license or valid Physical Therapy Compact privilege authorizing practice in Wisconsin.
• Graduate of a CAPTE-accredited Physical Therapist education program or completion of another educational pathway accepted by the Wisconsin Physical Therapy Examining Board.
• Successful completion of the National Physical Therapy Examination (NPTE) and applicable Wisconsin licensure requirements, including the Wisconsin Statutes and Administrative Code Examination when required.
• Current CPR certification.
• Strong clinical assessment, leadership, communication, organizational, and documentation skills.
• Ability to coordinate rehabilitation services across Greendale and Waukesha while maintaining quality resident care.
• Experience in geriatric rehabilitation, assisted living, skilled nursing, or related rehabilitation settings is preferred.
