Home Health Kauai is seeking a Physical Therapist to develop and implement a progressive rehabilitation program for patients in their homes, serving as case manager and supervising HHA and PTA when appropriate.

The ideal candidate holds a Master’s or Doctorate in Physical Therapy, Hawaii license, and at least 2 years of relevant experience, with CPR certification and TB clearance. Relocation assistance and a $2,000 retention bonus are offered along with PTO and strong growth opportunities.

• Two years experience as a physical therapist.
• Valid Hawaii license to practice as a Physical Therapist.
• Current Basic Life Support CPR certification through the American Heart Association
• Current medical/health screening including 2 step TB clearance

Responsabilidades

• Serves as case manager and supervises home health aides and physical therapist assistants when appropriate.
• Evaluates assigned patients and admits those who meet agency and regulatory criteria.
• Plans, coordinates, and implements the appropriate treatment according to patient’s needs and performs ongoing reassessments.
• Develop Home Health Aide and PT Assistant care plans; delegates duties and supervises per policy.

The Position:

We are looking for a passionate Physical Therapist (PT) to develop and implement a progressive rehabilitation program for patients in their homes in accordance with the principles and practices of physical therapy and the client’s needs. Serves as case manager and supervises home health aides and physical therapist assistants when appropriate.

• Evaluates assigned patients. Admits those who meet agency and regulatory criteria.
• Plans, coordinates, and implements the appropriate treatment according to patient’s needs. Performs ongoing reassessments. Discharges patients when goals are met, or patient does not meet home health requirements.
• Develop Home Health Aide (HHA) and Physical Therapy Assistant (PTA) care plans. Delegates duties to HHA’s and PTAs (Physical Therapy Assistant) based on agency and regulatory policies. Supervises and provides input to HHA's and PTAs per agency policy.
• Assumes responsibility for case management of assigned patients. Consults with appropriate community and internal resources and makes appropriate interdisciplinary referrals.
• Performs patient care duties and/or treatments based on specific needs, including medical condition, age, and/or developmental status.

Requirements:

• Master’s Degree or Doctorate Degree in Physical Therapy
• Two years experience as a physical therapist, preferably in home health or other healthcare practice setting.
• Valid State of Hawaii license to practice as an Physical Therapist.
• Current Basic Life Support CPR certification through the American Heart Association
• Current medical/health screening including 2 step TB clearance
