Job summary

Ohana Pacific Health in Hawaii seeks a passionate Occupational Therapist to develop and implement a progressive home health rehabilitation program in Lihue. This role serves as the case manager and supervises Home Health Aides and COTAs when appropriate.

You will evaluate patients, plan and coordinate treatments, reassess, and discharge when goals are met, delegating care plans according to agency policy. Relocation assistance and a $2,000 retention bonus are available, with growth opportunities

Qualifications

• Accredited OT program recognized by NBCOT with fieldwork completed.
• Current TB clearance and health screening.
• Hawaii license to practice as an Occupational Therapist.
• CPR certification (American Heart Association).
• Two years of OT experience, preferably in home health.

Responsibilities

• Evaluate assigned patients and admit those meeting criteria.
• Plan, coordinate and implement treatments; reassess and discharge when goals met.
• Develop Home Health Aide (HHA) and Certified Occupational Therapy Assistant (COTA) care plans; supervise staff.
• Manage caseload and coordinate referrals with interdisciplinary team.

The Position:

We are looking for a passionate Occupational Therapist (OT) to develop and implement a progressive rehabilitation program for patients in their homes in accordance with the principles and practices of occupational therapy and the patient’s needs. This position serves as the case manager and supervises home health aides and certified occupational therapy assistants when appropriate.

• Evaluates assigned patients. Admits those who meet agency and home health criteria.
• Plans, coordinates, and implements the appropriate treatment according to patient’s needs. Performs ongoing reassessments. Discharges patients when goals are met, or patient does not meet home health requirements.
• Develop Home Health Aide (HHA) and Certified Occupational Therapy Assistant (COTA) care plans. Delegates duties to HHA’s and COTA’s based on agency and regulatory policies. Supervises and provides input to HHA's and COTA's per agency policy.
• Assumes responsibility for case management of assigned patients. Consults with appropriate community and internal resources and makes appropriate interdisciplinary referrals.

Requirements:

• Have successfully completed and graduated from an accredited occupational therapy professional program recognized by NBCOT and have completed all fieldwork requirements
• Current medical/health screening including 2 step TB clearance.
• Valid State of Hawaii license to practice as an Occupational Therapist.
• Current Basic Life Support CPR (cardiopulmonary resuscitation) certification through the American Heart Association
• Two years of experience as an occupational therapist, preferably in home health or other healthcare practice setting.
