Job summary

Ohana Pacific Health in Honolulu is seeking a passionate Occupational Therapist to develop and implement a progressive rehabilitation program for patients at home, in line with occupational therapy principles and the patient’s needs. This role serves as the case manager and may supervise Home Health Aides and COTAs.

You will evaluate patients, plan and coordinate treatments, perform re-assessments, discharge when goals are met, and supervise care plans for HHA and COTA staff.

Qualifications

• Must have graduated from an NBCOT-recognized Occupational Therapy program and completed fieldwork requirements.
• Current medical health screening including 2-step TB clearance.
• Valid Hawaii license to practice as an Occupational Therapist.
• Current Basic Life Support CPR (CPR) certification through the American Heart Association.
• Two years of experience as an Occupational Therapist, preferably in home health or other healthcare setting.

Responsibilities

• Evaluates assigned patients. Admits those who meet agency and home health criteria.
• Plans, coordinates, and implements the appropriate treatment according to patient’s needs. Performs ongoing reassessments. Discharges patients when goals are met, or patient does not meet home health requirements.
• Develop Home Health Aide (HHA) and Certified Occupational Therapy Assistant (COTA) care plans. Delegates duties to HHA’s and COTA’s based on agency and regulatory policies. Supervises and provides input to HHA's and COTA's per agency policy.
• Assumes responsibility for case management of assigned patients. Consults with appropriate community and internal resources and makes appropriate interdisciplinary referrals.

The Position:

Requirements:

• Have successfully completed and graduated from an accredited occupational therapy professional program recognized by NBCOT and have completed all fieldwork requirements
• Current medical/health screening including 2 step TB clearance.
• Valid State of Hawaii license to practice as an Occupational Therapist.
• Current Basic Life Support CPR (cardiopulmonary resuscitation) certification through the American Heart Association
• Two years of experience as an occupational therapist, preferably in home health or other healthcare practice setting.
