Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patients' needs related to mobility, endurance, pain, and other medical conditions. Based on this assessment, the PT develops treatment plans, performs interventions to improve patient well-being, and monitors progress.

• Evaluate the patient's functional status and therapy needs; assist in developing the care plan with the physician
• Monitor and report patient responses and condition changes to the healthcare team
• Provide treatments to relieve pain, restore function, and maintain maximum mobility following accepted standards
• Educate patients, families, and healthcare team members about the care plan, limitations, and progress

Education and Experience

Licensure Requirements

• Current Physical Therapy license in Kentucky
• Current CPR certification
• Valid driver's license, vehicle insurance, and reliable transportation
