Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to work in patients' homes on a part-time/per diem basis. You will evaluate, plan, and provide PT services to help clients improve strength, balance, mobility, and safety within familiar home environments.

The role offers competitive pay per visit ($90-$100) plus mileage reimbursement, flexible scheduling, and ongoing training.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certified.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
