Job summary

BrightStar Care of Concord, Lexington, and Woburn is seeking a licensed Physical Therapist to provide in-home physical therapy services on a part-time/per diem basis. You will assess clients, develop personalized plans, and deliver PT interventions in home settings to improve strength, balance, mobility, and safety.

Requirements include an active MA PT license, DPT or MPT, CPR certification, and reliable transportation.

Qualifications

• Active MA PT license is required.
• DPT or MPT from an accredited program.
• CPR certification required.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle and valid driver's license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
