The Physical Therapist is responsible for planning and delivering physical therapy services to patients in their homes in accordance with the Plan of Care and agency policies. This role requires specialized knowledge, clinical judgment, and the ability to work independently while ensuring high-quality patient care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current state license to practice as a Physical Therapist.
• Minimum 2 years of healthcare experience; home health preferred.
• Valid CPR certification.
• Strong communication and interpersonal skills.
• Ability to work independently.
• Valid driver’s license and insured vehicle per state/agency requirements.

Essential Functions & Responsibilities

• Conducts physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluations.
• Completes initial and comprehensive assessments (including OASIS) within 48 hours of referral unless otherwise arranged.
• Develops and updates individualized therapy care plans based on patient needs and agency policy.
• Documents all assessments, interventions, and outcomes accurately and submits within agency timelines.
• Communicates effectively with physicians, case managers, and interdisciplinary team members.
• Provides patient and caregiver education tailored to the patient’s condition, environment, and learning ability.
• Participates in care conferences, discharge planning, and interdisciplinary team meetings.
• Contributes to quality improvement initiatives and clinical record reviews.
• Ensures compliance with safety and infection control protocols.
• Supervises Physical Therapy Assistants and Home Health Aides as required.
• Performs in-home supervision and evaluations of care provided by Physical Therapy Assistants.
• Maintains compliance with state, federal, CHAP, and JCAHO standards.
• Engages in ongoing professional development and attends required training sessions.

Supervision of Physical Therapy Assistants

• Provides instruction and supervision for Physical Therapy Assistants in designated aspects of patient care.
• Remains available by phone during all treatment sessions conducted by assistants.
• Conducts on-site supervision every 4–6 visits, at least once every two months.
• Performs functional assessments and updates the Plan of Care as needed.
