All Care and Comcare Home Health is seeking a compassionate and motivated Physical Therapist to join our growing team. This Field Physical Therapist role offers the opportunity to provide skilled therapy and individualized care in patients' homes with a flexible schedule and supportive team environment.

You will perform SOC, ROC, Recert, and DC evaluations, develop patient-centered goals, educate families, and collaborate with an interdisciplinary team to ensure high-quality home health services

Qualifications

• Must hold an active California Physical Therapy license.
• Graduate of an accredited Physical Therapy program.
• Excellent documentation and organizational skills.
• Willingness to travel to patient homes across service area.
• Flexible working locations within service area.

Responsabilités

Field Physical Therapist

Key Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.
