Job summary

Ahava Therapy Services is seeking a Full-Time Physical Therapist to join the in-house rehabilitation team at Wexford House, a 173-bed facility in Kingsport, TN. You will evaluate residents, develop individualized plans, supervise therapy staff, and document progress to support meaningful daily activity and quality of life.

Qualifications

Bachelor’s or Master’s degree in Physical Therapy.
Current PT license in the state of practice.
BLS/CPR certification (current or obtained during orientation).
Strong communication, teamwork, and organizational skills.

Responsibilities

Review resident information and evaluate its impact on assessment and treatment.
Conduct and document thorough functional assessments using standardized procedures.
Develop and implement individualized treatment plans with achievable goals.
Comply with evaluation, treatment, and documentation guidelines; maintain timely, accurate records.
Instruct and supervise therapy assistants as needed.
Enter treatment and progress notes for each session.
Revise treatment plans as needed throughout the course of care.
Document treatment in discharge summaries.
Collaborate with nursing staff, residents, and families to maximize functional abilities.
Educate residents and caregivers on home exercise and therapy continuation.
