Legacy Healthcare Services is seeking a Physical Therapist to join a collaborative rehabilitation team. You will provide individualized rehabilitative services to senior residents under medical orders, and contribute to coordinated care through plan development and case conferences.

The position emphasizes one-on-one, function-based treatment and opportunities for professional growth in a not-for-profit network serving hundreds of senior living communities nationwide.

Qualifications

• This role requires a degree in Physical Therapy and an active state license.

Responsibilities

• Provide rehabilitative services to residents under medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the plan of care for patients.
• Treat residents one-on-one with individualized, function-based sessions.

• Provide rehabilitative services to patients under specific medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.

• Degree in area of discipline
• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment
