Job summary

Tallgrass Healthcare Campus is seeking a compassionate Physical Therapist to help residents regain strength, mobility, independence, and quality of life in a skilled nursing and rehabilitation setting.

You will join a collaborative rehab and nursing team, evaluate and treat cognitive/ADL disorders, and develop individualized care plans to maximize recovery, with opportunities for FullTime, PartTime, or PRN schedules.

Qualifications

• Bachelor’s degree or higher in Physical Therapy or related field.
• Active state Physical Therapist license.
• 2+ years of experience in a skilled nursing facility preferred.
• Strong clinical reasoning and documentation skills.
• Passion for working with seniors and improving functional outcomes.
• Ability to work collaboratively in a multidisciplinary team.

Responsibilities

• Evaluate and treat residents with cognitive and/or ADL physical functioning disorders.
• Develop and implement individualized plans of care using evidence-based practice.
• Provide skilled, medically necessary therapy to maximize recovery and functional independence.
• Administer rehabilitation interventions including therapeutic exercises, activities, ADL retraining, modalities, and assistive devices.
• Monitor resident progress and adjust treatment plans accordingly.
• Educate residents, families, and caregivers regarding treatment plans and outcomes.
• Communicate effectively with the interdisciplinary team regarding progress and barriers.
• Complete accurate, timely clinical documentation and treatment minutes.
• Provide thorough discharge summaries per regulatory and clinical standards.
• Participate in care conferences, rehab meetings, and required trainings.
• Supervise and support PTAs, rehab aides, and students as assigned.
• Maintain compliance with state practice acts, professional ethics, and facility policies.

This position reports directly to the Director of Rehabilitation Services.
