Job summary

Tallgrass Healthcare seeks an Occupational Therapist to join our rehabilitation team in a skilled nursing setting. You will evaluate, treat, and guide residents to regain independence and improve daily functioning, working with a collaborative interdisciplinary team.

The role involves developing care plans, providing evidence-based OT interventions, educating families and staff, and ensuring proper documentation and regulatory compliance.

Qualifications

  • Bachelor’s degree or higher in Occupational Therapy or related field.
  • Active Kansas Occupational Therapist license.
  • 2+ years of experience in a skilled nursing facility preferred.

Responsibilities

  • Evaluate and treat residents with cognitive and/or ADL physical functioning disorders.
  • Develop, implement, and update individualized plans of care using evidence-based practice.
  • Provide skilled, medically necessary OT interventions to promote recovery and functional independence.
  • Administer rehabilitation techniques including ADL retraining, therapeutic activities, exercises, modalities, and assistive devices.
  • Monitor resident progress and adjust treatment plans as needed.
  • Evaluate treatment outcomes and recommend modifications or referrals.
  • Educate residents, families, caregivers, and staff regarding treatment goals and progress.
  • Communicate with the Director of Rehabilitation Services and interdisciplinary team.
  • Complete timely, accurate documentation and record treatment minutes per regulatory requirements.
  • Prepare comprehensive discharge summaries.
  • Participate in care conferences, rehab meetings, trainings, and in-services.
  • Supervise and support OT assistants, rehab aides, and students as assigned.
  • Maintain compliance with NBCOT, AOTA, Kansas practice acts, ethical standards and facility policies.

This position reports directly to the Director of Rehabilitation Services.

What We’re Looking for in YOU
  • Bachelor’s degree or higher in Occupational Therapy or related field
  • Active Kansas Occupational Therapist license
  • 2+ years of experience in a skilled nursing facility preferred
  • Strong clinical judgment, organization, and communication skills
  • A passion for helping seniors maintain independence and dignity
