Confluent Health is seeking a compassionate Physical Therapist to join our outpatient clinic team. The role welcomes new grads or experienced therapists focused on helping patients achieve optimal function and quality of life.

The schedule is Mon-Fri, with compensation ranging from $72,000 to $90,000 and a potential sign-on bonus up to $10,000. We support mentors, development pathways, and innovative tools to advance care.

Qualifications

• Licensed Physical Therapist (or ability to obtain licensure in the state of practice)

Responsibilities

• Evaluate, diagnose, and treat patients with musculoskeletal, orthopedic, neurological, and sports-related conditions.
• Develop personalized treatment plans based on thorough assessments and patient goals.
• Implement evidence-based therapeutic interventions to improve mobility, strength, and overall function.
• Monitor patient progress and modify treatment plans as necessary.
• Educate patients on self-management techniques, home exercises, and lifestyle modifications to enhance long-term recovery.
• Collaborate with other healthcare professionals to ensure coordinated and comprehensive care.
• Maintain accurate and timely patient documentation in compliance with clinic policies and regulatory requirements.
• Provide guidance and mentorship to support staff and assist with ongoing professional development
