Transitions At Home for Western Wisconsin is seeking a Full-Time Physical Therapist to join our home health team. You will deliver skilled therapy in patients' homes, building individualized care plans and collaborating with an interdisciplinary team to restore mobility and independence.

Eligible candidates hold an active Wisconsin PT license, CPR certification, and a valid WI driver's license with car insurance.

Responsibilities

• Develop and revise home care plans with patients and families.

Your Role:

As a Home Health Physical Therapist, you’ll deliver skilled therapy services based on individualized care plans and physician orders. You’ll work closely with the interdisciplinary team to support patients in regaining strength, mobility, and independence at home.

Key Responsibilities:

• Develop and revise care plans
• Perform skilled physical therapy procedures
• Educate patients and families on home programs and daily activities
• Collaborate with physicians and the care team
• Participate in IDT meetings, in-service training, and QA initiatives

Qualifications:

• Active Wisconsin Physical Therapist license
• CPR certification
• Valid WI driver’s license & car insurance
• Comfortable working independently in a home care environment
