Job summary

The Visiting Nurse Association (VNA) in Iowa City seeks a Part-Time Physical Therapist to join our home health team. Provide one-on-one care in patients' homes and help them regain independence.

This 20-hour-per-week role offers flexible scheduling, with benefits prorated. You will evaluate, treat, educate, and collaborate with a multidisciplinary team across the community.

Qualifications

• Graduate from an approved School of Physical Therapy and Iowa-licensed.
• One year of PT experience preferred, home-health preferred.
• Strong knowledge of anatomy, physiology and rehabilitation.

Responsibilities

• Conduct comprehensive patient evaluations and develop plans.
• Provide interventions to improve strength, balance, mobility, and function.
• Monitor progress and adjust plans to maximize outcomes.
• Educate patients and caregivers on safety and home exercises.

As a Home Health Physical Therapist, You Will

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Provide therapeutic interventions designed to improve strength, balance, mobility, and overall function.
• Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
• Educate patients and caregivers on safety, injury prevention, body mechanics, and home exercise programs.
• Collaborate closely with physicians, nurses, occupational therapists, caregivers, and other healthcare professionals.
• Maintain timely, accurate documentation and clinical records.
• Promote patient independence and quality of life through compassionate, evidence-based care.
• Follow all agency policies, safety standards, and regulatory requirements during home visits.

This is a part-time position (20 hours per week) Monday-Friday 8:00 a.m. - 5:00 p.m.

Requirements

• Graduate from an approved School of Physical Therapy, licensed to practice in the State of Iowa.
• One year of PT experience, previous experience providing direct patient care in a healthcare or home health setting preferred.
• Knowledge of anatomy, physiology, kinesiology, and rehabilitation principles.
• Excellent communication and interpersonal skills.
• Strong clinical reasoning and problem-solving abilities.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Commitment to providing compassionate, patient-centered care.
• Valid driver's license, reliable transportation, and required auto insurance. All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law and a negative finding on the Office of Inspector General’s Excluded Provider List.
• Work is performed in clients' homes with the employee driving from home to home during the work period. The clients' homes are throughout the service area, including Iowa City, small towns and rural areas. The driving requirements include in-town and highway driving in good and inclement weather.
• All professional staff will use Ipad to document their work in the client homes or at the clinic sites.
