Job summary

American Senior Communities is seeking a PRN Physical Therapist for Rosewalk Village Indy / Community Nursing & Rehab. The PT provides skilled physical therapy services in accordance with established policies and procedures for ASC Therapies & Wellness settings.

You will evaluate patients, develop initial and ongoing treatment plans, discharge plans, supervise PTAs, contribute to case management, and maintain a positive interaction with patients and staff while adhering to state regulations.

The Physical Therapist implements skilled physical therapy services for patients in all ASC Therapies & Wellness settings, in accordance with the principles and practices of physical therapy and within the policies and procedures.
