Job summary

VNA of Hanover & Spring Grove is seeking a dedicated Physical Therapist to join our home health team in Hanover, PA. This full-time role offers a 5,000 sign-on bonus and a mission-driven environment focused on compassionate in-home care.

You will conduct thorough home assessments, develop personalized treatment plans, and educate patients and families to improve mobility and independence while collaborating with an interdisciplinary team.

Qualifications

• Pennsylvania Physical Therapist license is required.
• Degree in Physical Therapy from an accredited program.
• CPR certification required.

Responsibilities

• Assess patients and develop personalized home-based treatment plans.
• Deliver skilled physical therapy services in patients' homes to improve mobility and function.
• Educate patients and families on exercises to support recovery and prevent injury.

Key Responsibilities

• Conduct thorough assessments to evaluate patients' physical conditions and develop personalized treatment plans.
• Provide skilled physical therapy services to patients in their homes, focusing on improving mobility, strength, and function.
• Educate patients and their families on exercises and techniques to enhance recovery and prevent further injury.
• Collaborate with an interdisciplinary team to ensure comprehensive and coordinated patient care.
• Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
• Maintain accurate and timely documentation of patient care and progress.

Qualifications

• Valid Pennsylvania Physical Therapist license.
• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Must possess a bachelor’s or master’s degree in physical therapy approved by an accredited organization: The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, The Council on Medical Education of the American Medical Association and The American Physical Therapy Association.
• CPR certified.
• Previous experience in home health or hospice care is preferred, but not required.
• Strong communication and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
• Compassionate, patient-focused approach to care.
