Job summary

Fifth Star Care Management is seeking a dedicated Physical Therapist (PT) to join our IN-HOUSE therapy team. New grads welcome and a generous $8,000 sign-on bonus are available for full-time positions.

We focus on patient-centered care to improve mobility, strength, independence, and quality of life for residents. The PT will provide assessments, develop plans, and collaborate with an interdisciplinary team.

Qualifications

• Master's (MPT) or Doctor of Physical Therapy (DPT) degree from an accredited program.
• Valid state licensure or eligibility for licensure as a Physical Therapist.
• Experience in a skilled nursing setting preferred but not required.
• Familiarity with discharge planning processes to facilitate smooth transitions for patients.
• Experience treating geriatric populations is a plus.

Responsibilities

• Assess patients' physical abilities and limitations through comprehensive evaluations.
• Develop individualized treatment plans based on patient needs, goals, and clinical findings, incorporating evidence-based practices.
• Implement therapeutic exercises, functional activities, and modalities to improve mobility, strength, balance, and function.
• Educate patients and families on treatment plans, home exercises, safety strategies, and self-management techniques.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with interdisciplinary team for comprehensive care and discharge planning.
• Maintain accurate documentation of evaluations, interventions, and outcomes in line with regulatory standards.
• Participate in care planning and interdisciplinary meetings as needed.

Position Summary

We are looking for a Physical Therapist who is passionate about providing high-quality, patient-centered care and helping residents improve mobility, strength, physical function, independence, and overall quality of life.

Responsibilities

• Assess patients' physical abilities and limitations through comprehensive evaluations
