Job summary

Park Place Transitional Care and Rehabilitation is seeking a Physical Therapist to join our in-house therapy team. The role focuses on evaluating and treating residents with physical functioning disorders and wound care needs, following regulatory and clinical standards.

We offer a $20,000 sign-on bonus and a collaborative, family-like work environment where all therapy staff share the same goals and work directly with the facility. New grads are welcome to apply.

Qualifications

• Must hold a current Physical Therapist license.
• Open to PTs at all experience levels; new grads welcome.
• In-house therapy team with collaborative goals.

Responsibilities

• Screen/evaluate patients with physical functioning disorders and/or wound care; develop appropriate plan of care.
• Provide rehabilitative, skilled and medically necessary treatment per standards.
• Administer procedures as part of rehab: manual techniques; ambulation; therapeutic exercises; modalities; devices; wound care.
• Provide counseling to patients and families and other care providers related to physical disorders.
• Provide comprehensive discharge summaries of services per regulatory requirements.

Duties

• Effectively screens/evaluates patients and/ with physical functioning disorders and/or wound care and develops appropriate plan of care following all regulatory and clinical practice standards.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders and/or wound care per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: manual techniques; ambulation; therapeutic exercises; modalities; use of supportive and assistive devices; wound care; etc.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
