Job summary

Total Care Therapy in Granger, IN is seeking a compassionate Physical Therapist to evaluate, develop, and implement individualized treatment plans that improve mobility, restore function, reduce pain, and maintain independence for residents in assisted living and independent living communities.

You'll join a therapist-owned, clinician-first culture that emphasizes mentorship, collaboration with an interdisciplinary team, and meaningful work with older adults.

Qualifications

• Licensed Physical Therapist in the state of Indiana (or eligible)
• Current CPR/BLS certification
• Valid driver's license and reliable transportation
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled PT treatments including therapeutic exercise and gait/balance training.
• Reassess progress and modify plans as needed.
• Educate residents and staff on treatment plans and home exercises.
• Collaborate with interdisciplinary team for coordinated care.
• Maintain a productive clinical caseload with high-quality services

About the Role

We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
