Job summary

Total Care Therapy in Indiana is seeking a compassionate Physical Therapist to evaluate and treat residents in assisted living and related communities, developing personalized plans to improve mobility, reduce pain, and restore independence.

You will supervise PTAs, document in EMR, and collaborate with an interdisciplinary team, with opportunities for leadership growth as the organization expands across Indiana.

Qualifications

• Current State PT License.
• CPR/BLS Certification is current.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.
• Leadership, supervisory, or Director of Rehabilitation experience preferred, but leadership potential will be considered.
• Strong communication, organizational, and leadership skills.

Responsibilities

• Provide skilled physical therapy services according to established plans of care.
• Deliver therapeutic exercise, gait training, balance activities, transfer training, and mobility interventions.
• Monitor patient progress and modify treatment plans as needed to achieve functional goals.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents and caregivers on home exercise programs, fall prevention, and safety strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering compassionate, high-quality therapy services.
• Act as the main liaison between leadership and the facility.
• Oversee staff performance and support professional development.
• Manage administrative and operational tasks across therapy services.
• Coordinate meetings, training, and events for staff and residents.
• Maintain communication with residents, families, and leadership.
• Drive referrals for all disciplines.

About the Role
