PASCO/SW Home Health & Hospice is seeking an LPN to provide hands-on patient care in home settings under RN supervision. You will ensure comfort, safety, and positive outcomes for patients while documenting progress and following care plans.

The role emphasizes flexibility, a supportive team culture, and opportunities for ongoing training and certification. A valid Colorado LPN/LVN license and CPR certification are required, with a driver’s license and reliable transportation preferred.

Qualifications

• Graduate of an accredited practical vocational nursing program.
• Current Colorado LPN/LVN license.
• 3 years nursing experience preferred (Home Health or Med-Surg a plus).
• Current CPR certification.

Responsabilités

• Deliver compassionate direct patient care in accordance with state regulations.
• Implement care plans developed by the RN and document progress accurately.
• Monitor patient conditions and communicate observations to the RN and physician.

Pay Rate $26.00 - $ 33.00
