Gundersen Health System seeks a full-time Physical Therapist to provide care in both inpatient and outpatient settings. You will work with patients across a wide range of conditions at Emplify Health by Gundersen, Moundview Memorial Hospital & Clinics and the Extended Care Center in Friendship, WI.

The role involves 40 hours/week, 8-hour shifts, and occasional weekend coverage, with competitive pay and a comprehensive benefits package designed to support professional growth and work-life balance.

Qualifications

• Graduate from an accredited program for Physical Therapy.
• Current licensure as a Physical Therapist in the State of WI.
• CPR Certification (or ability to obtain within 3 months).

Responsibilities

• Work with patients in inpatient and outpatient settings.
• Assess physical and psychosocial needs of patients.
• Plan, develop, and implement multidisciplinary plan of care.
• Evaluate patients’ responses to care.
• Maintain patient records including evaluations, progress notes, and discharge summaries.

We are seeking a full-time Physical Therapist to provide care for patients in both inpatient and outpatient settings. In this role, you will work with individuals experiencing a wide range of conditions at Emplify Health by Gundersen, Moundview Memorial Hospital & Clinics and the Extended Care Center in Friendship, WI.

What you will do/job specifics

• Full-time 40 hours/week
• 8 hour shifts, 7am-3:30pm and 8am-4:30pm
• Occasional coverage on weekends
• Work closely with patients to assess their physical and psychosocial needs.
• Planning, development, and implementation of multidisciplinary plan of care
• Evaluation of patients’ responses to care.
• Maintain patient records including initial evaluations/assessments, progress notes, and discharge summaries.

What’s Required/What you need
