TEEMA is hiring a Physical Therapist to assess and treat elderly patients in the Fresno area, splitting time between clinic and field.
You will develop individualized care plans, educate patients and families, and collaborate with a multidisciplinary team to promote independence and safety at home. A California license and geriatric experience are preferred. Training is available for candidates with relevant experience.
The Physical Therapist plans, organizes, and delivers comprehensive physical therapy services to support elderly and frail patients in maintaining independence, safety, and quality of life within the community. This role involves patient assessment, individualized treatment planning, hands-on therapy, and education of patients, caregivers, and families, while collaborating closely with a team to ensure coordinated, high-quality care across multiple settings.
Chronic conditions are long-term health issues requiring ongoing management, such as diabetes, hypertension, pulmonary disease, kidney failure, multiple sclerosis, and Parkinson's disease.
The overall goal is to keep patients as independent as possible, ensure they have access to all necessary resources, and reduce rehospitalizations by maintaining their health and safety in their homes.
Candidates must also be comfortable responding to emergencies, acting quickly, and utilizing strong critical thinking skills.
