Job summary

TEEMA seeks a Licensed Physical Therapist to evaluate and assess geriatric patients, determining mobility and functional abilities while planning personalized exercise programs for maintenance therapy. The role ensures high standards of patient care through comprehensive PT programs.

The position is Monday–Friday with no weekends. Responsibilities include data collection, goal-oriented care plans, treatment administration, patient-family education, supervision of aides, and active participation

Qualifications

• Licensed Physical Therapist
• 2 years of Physical Therapist experience
• Newly Licensed welcomed
• 1 year of experience with frail/elderly population preferred
• Clinical competency with geriatric population
• Reliable transportation and valid driver’s license with auto insurance

Responsibilities

• Collect and document patient data, identify problems, and set priorities
• Develop care plans with measurable goals and interdisciplinary collaboration
• Provide and document prescribed treatments, ensuring adherence to standards
• Educate patients and families on rehabilitation needs and interventions
• Supervise aides and staff, ensuring cost-effective rehabilitation practice
• Participate in interdisciplinary team meetings and care planning
• Effective teamwork and communication with the team
• Other duties as assigned
