• Graduated from an accredited Physical Therapy program.
• Active Physical Therapy license in the state of practice.
• Ability to conduct physical therapy assessments of referred patients and provide direct patient care.
• Proficient in PCC EMR documentation; CPR certification is required.

Responsibilities

• Provide direct patient care and perform PT assessments to assess medical conditions and functional capabilities.
• Develop and implement rehabilitation plans in collaboration with the healthcare team.
• Maintain accurate PCC EMR documentation and ensure compliance with licensing requirements.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.
