Job summary

Lafayette Center in New Hampshire is seeking a Physical Therapist (PT) to deliver high-quality physical therapy services in the Rehabilitation department. The PT will collaborate with residents, families, nursing staff, and the interdisciplinary team to promote mobility, safety, and quality of life.

Responsibilities include evaluating residents, creating individualized treatment plans, and documenting progress while adhering to physician orders and facility policies.

Qualifications

• Current Physical Therapist license in New Hampshire (NH).
• Graduate of an accredited Physical Therapy program.
• Strong communication, organization, and clinical skills.
• Experience in skilled nursing or long-term care preferred.
• Ability to work effectively as part of an interdisciplinary team.

Job description

Position: Physical Therapist (PT)
Department: Rehabilitation
Reports To: Director of Rehabilitation

Position Summary

The Physical Therapist is responsible for providing quality physical therapy services to residents in accordance with individualized treatment plans, physician orders, facility policies, and applicable regulatory standards. The PT works closely with residents, families, nursing staff, and the interdisciplinary team to promote mobility, independence, safety, and overall quality of life.

Essential Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.

• Provide skilled physical therapy services consistent with physician orders and the resident’s plan of care.

• Treat residents using appropriate therapeutic exercises, functional activities, mobility training, and other skilled interventions.

• Monitor resident progress and modify treatment plans as clinically appropriate.

• Document evaluations, treatments, progress, and discharge information accurately and timely.

• Communicate changes in resident condition and treatment progress to the appropriate clinical team.

• Participate in care planning and interdisciplinary meetings as needed.

• Educate residents, families, and caregivers regarding exercises, mobility, positioning, and safety.

• Assist with discharge planning and provide recommendations for continued care when appropriate.

• Maintain compliance with facility policies, Medicare/Medicaid requirements, and applicable federal and state regulations.

• Maintain professional standards of conduct, confidentiality, and resident rights.

Qualifications

• Current Physical Therapist license in the State of New Hampshire.

• Graduate of an accredited Physical Therapy program.

• Current CPR certification.

• Experience in skilled nursing or long-term care preferred.

• Strong communication, organization, and clinical skills.

• Ability to work effectively as part of an interdisciplinary team.
