ABOUT OUR CLIENT

Our client is a 25-bed, non-profit community hospital designated as a Critical Access Hospital. Located in Wolfeboro, NH, we provide primary care, diagnostic testing and specialty services in the region. The hospital serves a year-round population of 30,000 residents and approximately 120,000 seasonal residents and visitors from the Eastern Lakes Region and surrounding towns including Alton, Brookfield, Effingham, Freedom, Madison, Moultonborough, New Durham, Ossipee, Sanbornville, Sandwich, Tamworth, Tuftonboro, Wolfeboro and nearby areas.

The hospital and its professional medical staff have a reputation for excellence in primary care, inpatient and outpatient surgery, medical imaging and diagnostics, gynecology, orthopedics, pediatrics, physical therapy, cardiac and occupational rehabilitation and emergency services. WHO YOU ARE

Under the general supervision of the Director of Rehabilitation and Diagnostic Services, the Physical Therapist is responsible for administering Physical Therapy services, including evaluations, establishment/modification of treatment plans, provision/supervision of treatment, and documentation for inpatients and outpatients in line with departmental and facility policies. The role aims to restore function, alleviate pain and prevent disabilities by planning and delivering medically prescribed physical therapy.

This is a full-time position working 30 hours per week. This position is eligible for participation in medical benefits.

RESPONSIBILITIES
Clinical Evaluation
  • Validate that a provider's prescription is present and understood prior to evaluation and treatment. Conduct evaluation and treatment according to the provider\'s order; contact the provider if clarification is needed.
  • Perform evaluations and reevaluations according to departmental policies.
  • Evaluate effects of physical therapy treatments and the fit of prosthetic and orthotic devices by observing and documenting progress; recommend adjustments as needed.
  • Use objective measures on all evaluations and documented findings. Develop measurable/functional goals based on these findings.
  • Develop a treatment plan with input from the patient and in accordance with the provider\'s order.
Treatment and Discharge Plan
  • Ensure treatment plans address the patient\'s needs including age, physical, psychological and learning considerations. Encourage patient participation in their plan of care and goals.
  • Apply a variety of therapeutic exercises, functional training, manual therapy techniques, assistive and adaptive devices, and physical agents and electrotherapeutic modalities.
  • Educate the patient on ongoing goals as they are achieved or modified and on impending discharge.
  • Complete discharge planning by consulting with physicians, nurses, social workers and other health care staff; contribute to patient care conferences.
  • Support continuation of the therapeutic plan after discharge by designing home exercise programs; instruct patients, families and caregivers; recommend assistive equipment and outpatient or home services as appropriate.
Quality and Customer Service
  • Greet and welcome all patients by name and on time.
  • Ensure documentation is complete and thorough in accordance with departmental policies and insurance requirements.
  • Complete and edit charges daily to ensure accuracy.
  • Participate in mandatory competencies and monthly chart audits.
  • Maintain professional and technical knowledge by attending educational activities and participating in professional networks.
  • Contribute to the development of physical therapy staff through information sharing and in-service training.
Other
  • Comply with federal, state, and local legal and professional requirements; stay informed about new legislation and advise management on needed actions.
  • Maintain awareness of insurance demands, restrictions and authorizations; educate patients about their insurance.
  • Teach/lead physical therapy students and other health professionals; educate providers about the scope of physical therapy services.
  • Direct and supervise Physical Therapy Assistants (PTAs), delegating patients and maintaining ongoing communication.
  • Collaborate with other health professionals for patient well-being.
  • Travel to off-site approved locations as needed to provide therapy services and wellness programs.
MINIMUM EDUCATION AND WORK EXPERIENCE
  • Education: Bachelor\'s and/or Master\'s and/or Doctorate degree in Physical Therapy from an APTA-accredited program.
  • Experience: 3-5 years in hospital and outpatient settings.
  • Licensed in NH by the NH State Board of Allied Health.
  • CPR certification.

  • Hours: 30 or 40 hours per week.
