Job summary

Visiting Nurse and Hospice for VT & NH is seeking a licensed Physical Therapist to provide home health services across VT and NH. The role emphasizes autonomous clinical decision-making, collaboration with a care team, and mentoring students and staff.

Responsibilities include comprehensive assessments, treatment planning, and education for patients and caregivers. Prior acute care experience is preferred; be prepared to coordinate care and refer when needed.

Qualifications

• Active PT license in VT and NH required.
• Bachelor's degree or higher.
• 1 year of experience as a physical therapist in hospital, clinic or home health agency.
• 2 years of acute care preferred.

Responsibilities

• Clinical assessment and treatment of patients referred by a licensed physician.
• Coordinate and communicate treatment activities with health care team members and the patient.
• Supervise Physical Therapy Assistant (PTA) and contribute to the professional development of new staff and health professional students.
• Evaluate patient’s needs for physical therapy and develop appropriate treatment plan.
• Provide appropriate care and treatment to patients based on physician care plan, agency policy, and professional standards of care.
• Instruct family members, PTA’s, Home Health Aides, and other caregivers in appropriate physical therapy techniques.
• Coordinate physical therapy progress with other caregivers and ensure appropriate physical therapy techniques.
• Refer patients to other professionals or agencies when appropriate.
