Job summary

Elara Caring in Fort Wayne, IN seeks a licensed Physical Therapist to provide high-quality home-based PT services. You will evaluate patients, develop and implement individualized treatment plans, and monitor progress in a home setting.

Responsibilities include collaboration with physicians and care teams, maintaining accurate clinical documentation, supervising therapy assistants when needed, and supporting patients and families through therapy and mobility goals.

Qualifications

• Graduate of an APTA approved physical therapy program.
• Current, unrestricted PT license valid for the state.
• Minimum one year of healthcare facility experience.
• Must possess a valid driver's license or reliable transportation to travel to patient homes.
• Ability to frequently lift up to 50 pounds during patient handling.
