Job summary

Angels Care Home Health is seeking a licensed Physical Therapist for a full-time role in Clermont, FL. You will plan, implement, and evaluate patient care plans to restore or maintain well-being, based on referrals and evaluations.

Collaboration with an interdisciplinary team is essential in delivering quality care. Monday through Friday schedule with benefits including health, dental, and vision coverage, PTO, CEU access, a company-matched 401(k), and opportunities for advancement.

Qualifications

• Current state license as a Physical Therapist.
• Minimum of 2 years of experience as a Physical Therapist.
• CPR certification and Hepatitis profile.
• Knowledge of durable medical equipment (DME).

Responsibilities

• Plan, implement, and evaluate patient care plans to restore or maintain patient well-being.
• Plan, organize, and conduct physical therapy treatment programs based on medical referral and evaluation.
• Collaborate with interdisciplinary team to optimize patient outcomes.
