Job summary

Award Care is seeking Physical Therapists to join our family and deliver compassionate PT services in a nursing home setting. Full time or Part time positions start at $42 per hour, based on experience.

PRN roles start at $50 per hour, with compensation depending on experience. We offer competitive benefits and a supportive team environment.

Qualifications

• Must be a graduate of a School of Physical Therapy.
• Florida PT license required.
• CPR certification required or equivalent.

Responsibilities

• Develop Plan of Treatment for each patient based on initial and ongoing evaluations, with short- and long-term goals.
• Implement Plan of Treatment on a consistent schedule and adjust as residents progress.
• Document residents’ progress according to medical and facility procedures.
• Evaluate mobility, strength, and current level of resident function, setting attainable goals.
• Report progress to residents, caregivers, staff and involved professionals.
• Assist other staff to provide care and maintain a clean, effective rehab environment.

Major Responsibilities

• Develop Plan of Treatment for each patient based on initial and ongoing evaluations including short- and long-term goals.

• Implement Plan of Treatment on a consistent schedule and adjust this schedule and plan according to residents’ progress.

• Document residents progress according to approved medical documentation and facility documentation procedures.

• Evaluate mobility, strength, and current level of resident function, while setting goals to improve in these areas that are scaled to residents’ abilities and realistically attainable.

• Report resident progress to resident, resident caregivers and loved ones, facility staff and healthcare professionals involved in ongoing treatment/care of resident.

• Assist other facility staff members as needed to provide care and create a clean, orderly, and effective environment for rehabilitation and physical therapy.

Qualifications

• Graduate of School of Physical Therapy

• Minimum of three months recent experience as a Physical Therapist working in a nursing home environment preferred.

• State of Florida Physical Therapy License

• CPR Certification
