Job summary

VNA Health Group is seeking a Per-Diem Physical Therapist to deliver in-home and community-based physical therapy in Bergen County, NJ. You will evaluate patients, develop care plans with physicians, monitor progress, and document changes while maintaining safety and attendance standards.

Applicants should hold a valid NJ Physical Therapy License, with 2–3 years of clinical or community experience preferred, plus strong communication and transportation capability.

Key Responsibilities:

• Evaluate and treat patients based on individual physical therapy needs
• Develop and adjust care plans in collaboration with physicians

What You Need to Succeed:

• Valid NJ Physical Therapy License
• 2-3 years of clinical or community experience preferred
• Strong communication, interpersonal, and computer skills
• Valid NJ driver’s license and reliable transportation

Working Conditions/Physical Demand:

Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation
