Job summary

Visiting Nurse Association Health Group is seeking a Full-Time Physical Therapist to provide in-home and community-based therapy in Monmouth County, NJ. You will evaluate, treat, and tailor care plans to help patients improve mobility and quality of life, all while collaborating with physicians and a supportive team.

• Valid NJ Physical Therapy License.
• 2–3 years of clinical or community experience preferred.

• Develop and adjust care plans in collaboration with physicians.
• Monitor patient progress and document changes in condition.
