Job summary

Delmar Center for Rehabilitation and Nursing seeks a dedicated Physical Therapist to evaluate residents and develop individualized treatment plans to improve mobility, strength, and independence while enhancing quality of life.

You will collaborate with an interdisciplinary team and educate residents and families on home exercises, with a focus on safety and compliance across state and facility regulations.

Qualifications

• Licensed Physical Therapist required in the state.
• Graduate of an accredited Physical Therapy program.
• Experience in skilled nursing or rehab settings preferred.
• Strong clinical, communication, and documentation skills.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide therapeutic exercises, manual therapy, and mobility training.
• Monitor and document resident progress and adjust care plans as needed.
• Collaborate with interdisciplinary team to ensure comprehensive care.
• Educate residents and families on treatment plans and exercises.
• Ensure compliance with state, federal, and facility regulations.
• Maintain timely documentation per facility policies.
• Participate in care plan meetings and team discussions.

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team in a skilled nursing facility. The Physical Therapist will evaluate residents and develop individualized treatment plans to improve mobility, strength, and overall functional independence while enhancing quality of life.

Key Responsibilities:

• Evaluate residents and develop individualized physical therapy treatment plans

• Provide therapeutic exercises, manual therapy, and mobility training
