As a Physical Therapist, you will evaluate and treat residents to improve mobility, functional independence, and overall quality of life. You will work closely with an interdisciplinary care team to develop and implement individualized treatment plans that support each resident's rehabilitation goals.
Evaluate residents and develop individualized treatment plans.
Help residents improve mobility, strength, balance, and functional independence.
Utilize therapeutic exercises, manual therapy techniques, stretching, and rehabilitation equipment.
Educate residents and families regarding treatment plans and recovery expectations.
Monitor resident progress and adjust treatment plans as needed.
Ensure proper and safe use of therapy equipment.
Supervise Physical Therapy Assistants as appropriate.
Maintain accurate documentation in accordance with regulatory and facility requirements.
Follow infection-control policies and safety procedures.
Collaborate with interdisciplinary team members to provide comprehensive resident care.
Graduate of an accredited Physical Therapy program.
Current New York State Physical Therapist License.
Experience in skilled nursing, rehabilitation, or long-term care settings preferred.
Strong communication and interpersonal skills.
