The Grand Rehabilitation and Nursing At Mohawk Valle in Mohawk, NY is seeking a Physical Therapist to join our clinical team. You will evaluate and treat residents to improve mobility and independence, collaborating with an interdisciplinary team to develop individualized plans.

This full-time position offers a competitive pay range of $44–$48 per hour, with a schedule primarily Monday to Friday and some weekend coverage, along with comprehensive benefits.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current New York State Physical Therapist License.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Help residents improve mobility, strength, balance, and functional independence.
• Utilize therapeutic exercises, manual therapy techniques, stretching, and rehabilitation equipment.
• Educate residents and families regarding treatment plans and recovery expectations.
• Monitor resident progress and adjust treatment plans as needed.
• Ensure proper and safe use of therapy equipment.
• Supervise Physical Therapy Assistants as appropriate.
• Maintain accurate documentation in accordance with regulatory and facility requirements.
• Follow infection-control policies and safety procedures.
• Collaborate with interdisciplinary team members to provide comprehensive resident care.
