Position Overview

We are seeking a Physical Therapist to provide restorative and rehabilitative Physical Therapy services to participants both in the facility and at home. The Physical Therapist will work alongside the Primary Care Physician, rehabilitation team, and other interdisciplinary team members to maximize participant independence, safety, and function.

• Conduct Physical Therapy assessments in both facility and home settings to assess participants' needs, preferences, and goals.
• Actively contribute to interdisciplinary team meetings to develop participant care plans.
• Deliver and document Physical Therapy interventions according to physician orders and/or as agreed in the care plans.
• Provide active and passive exercise, muscle re-education, gait, and functional training for Activities of Daily Living (ADLs), as well as modalities such as massage, heat, sound, water, light, or electricity.
• Supervise Therapy Assistants by offering training, instruction, and guidance for implementing Functional Maintenance Programs (FMP).
• Provide training and instructions to staff, participants, and caregivers/families regarding the use and care of therapeutic equipment.

Job Requirements

• Bachelor's Degree in Physical Therapy with at least five (5) years of experience.
• A Master's or Doctorate in Physical Therapy is preferred.
• Valid Physical Therapy License or proof of eligibility.
• At least two (2) years of Physical Therapy experience in a clinical setting, preferably with frail or elderly populations.
• If the Physical Therapy degree is earned outside the country, it must be evaluated for equivalency to a degree in Physical Therapy.
• CPR certification preferred and must be maintained.
