Position: Part Time / Weekend Physical Therapist Case Manager

Coverage Area: Elk Grove

In collaboration with other clinicians, you will evaluate and address the needs of individuals in their homes, helping them live safely and productively. Your goals include restoring function to prevent or minimize disability following disease or injury, and assisting clients in reaching their maximum performance within their capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs, and may supervise Physical Therapy Assistants (PTAs) in accordance with agency and regulatory standards.
