Job summary

City Scape Rehabilitation and Care Center in Denver, CO is seeking a caring and dynamic Physical Therapist to join our skilled nursing and rehabilitation team. You will help residents regain quality of life through personalized treatments in a supportive, patient‑centered environment.

Work Schedule

Full‑time and part‑time available.

Pay Rate

$50.00 – $62.00 per hour, depending on experience.

Job Summary

The Physical Therapist performs treatments and exercises to help patients recover from illnesses and injuries. Their main duties include using various treatment techniques to develop personalized treatment plans, and teaching patients exercise techniques to improve their health. This position reports to the Director of Rehabilitation Services.

Job Duties

• Consult with patients to learn about their physical condition and symptoms. Review their medical history and referrals from doctors or surgeons.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plans.
• Coach patient and teach them therapeutic massage to ease patients’ pain and provide stimulation to promote healing.
• Maintain patient records by keeping track of goals.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to the patient’s physical disorders.
• Effectively communicate with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy.
• Accurately record treatment minutes for services provided per all regulatory and clinical requirements.
• Participate in facility and company‑required trainings, in‑services, and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adhere to company‑required policy and procedures and code of conduct.
• Active state licensure.

Requirements / Experience

• Bachelor’s degree or higher in Physical Therapy or related field.
• Hold a current PT license in the state of Colorado.
• Experience in skilled nursing is preferred.
